Making an Appointment
To make an appointment, please call (970) 482-6456 or our regional
toll-free number by dialing (888) 699-6272. If you need to change
your appointment time, please call us as soon as possible so
we may accommodate your request.
Office Hours
The office hours of our Fort Collins clinic (which is also the
practice's main business office) are from 8 a.m. to 5 p.m. during
regular working days.

Emergencies
If you have a medical emergency, dial 911. One of our physicians
is available 24 hours a day, including weekends and holidays.

by dialing (888) 699-6272. The answering service will quickly
route your call.
Surgery Scheduling
If you need to make an appointment for a surgical procedure
or have any questions about your surgery, please call our surgery
coordinator at (970) 482-6456 or (888) 669-6272.
Prescription Refills
If a prescription from one of our physicians needs refilling,
please call your pharmacist several days before you run out
of medicine. The pharmacist will call our office on your behalf
to authorize the refill.
Insurance
We participate with Blue Cross/Blue Shield, Pacificare, TRICARE,
United Healthcare, Colorado Medicaid, Wyoming Medicaid, Nebraska
Medicaid, Medicare, and Workers' Compensation carriers, as well
as most insurance providers and employer groups. If you have
a question about our participation with your plan(s), please
call us. Due to the hundreds of insurance plans and various
choices within plans, we encourage patients to study their insurance
benefits. Our office will assist you as you make informed choices
about how best to administer your care.
Referrals
Your primary doctor will help you begin the referral process
with your insurance provider. Our practice will help you submit
a request for preauthorization for surgery if it is required.
If you do not come to our practice through a referral, you may
be solely responsible for payment.
Billing
We require co-payments and deductibles be paid at the time of
your office visit or prior to surgery. We accept cash or personal
check, as well as Visa or MasterCard credit cards. Payment plans
can be arranged through our billing office. Please call (970)
482-9880 for more information. Hospital or surgery center charges
will be billed to you separately by the facility. Depending
upon the services you receive, you may also get bills from radiology,
anesthesia, and pathology, as well as from your primary care
doctor if he or she assists during your surgery.Your payment
for these services should be sent to the appropriate offices,
not to our practice. |
